  16. This is a request under the Freedom of Information Act as amended (5 U.S.C.
  17. 552).

  19. I request a complete and thorough search of all filing systems and locations
  20. pertaining to me, including but not limited to files and documents captioned
  21. in, or whose captions include my name in the title.  This request specifically
  22. includes "main" files and "see references," including but not limited to
  23. numbered and lettered subfiles, 1A envelopes, enclosures behind files (EBFs),
  24. Bulky Exhibits, control files, and "JUNE" files.  I awant all records to be
  25. produced with the administrative markings and all reports to include the
  26. administrative pages.

  28. I wish to be sent copies of "see reference" cards, abstracts, search slips
  29. including search slips used to process this request, file covers, multiple
  30. copies of the same document if they appear in a file, and tapes of any
  31. electronic surveillances.  Please search, "DO NOT FILE" files, SAC safes,
  32. special file rooms, and offices of FBI officials and special agents.  I want
  33. all pages released regardless of the extent of excising, even if all that
  34. remains are the stationery headings or administrative markings.

  48. is (are) claimed for each passage or whole document denied.  Please provide a
  49. complete itemized inventory and a detailed factual justification of total or
  50. partial denial of documents.  Specify the number of pages in each document
  51. and the total number of pages pertaining to this request.  For "classified"
  52. material denied please include the following information:  the classification
  53. (confidential, secret or top secret); identity of the classifier; date or
  54. event for automatic declassification, classifacation review, or downgrading;
  55. if applicable, identity of official authorizing extension of automatic
  56. declassification or review past six years; and, if applicable, the reason
  57. for extended classification past six years.

  59. In excising material, please "black out" the material rather than "white out"
  60. or "cut out."  I expect, as provided by the Freedom of Information Act, that
  61. the remaining nonexempt portions of documents will be released.

  63. As I expect to appeal any denials, please specify the office and address to
  64. which an appeal should be directed.

  66. Please send a memo (copy to me) to the appropriate units in your office to
  67. assure that no records related to this request are destroyed.  Please advise
  68. of any destruction of records and include the date of and authority for
  69. such destruction.

  71. I wish to make it clear that I want all records in your office "identifiable
  72. with my request" even though reports based on those records have been sent
  73. to FBI Headquarters and even though there may be duplication between the
  74. two sets of files.  Please do not refer me to Headquarters.
